Summer Camp Paperwork

1. Parent/Guardian Consent Form
2. Camper Health History Form
3. Physical Exam Form –  Note: a physical within the past 12 months is required by the ACA
4. Camper Confidentiality Form
5. Medication Authorization Form  – All medications can be listed on the same form. List all medications, even over-the-counter medications (melatonin, lactaid, vitamins, allergy medication such as Zyrtec, etc), that will be sent to camp. This form must be signed by a licensed medical professional.
6. Rock Climbing/High Ropes Consent Form  – for campers age 11 and older

If your camper is participating in the following camps, be sure to complete the required additional forms:

1. Adventure Camp – Canoeing Form
2. Pioneer Camp – Canoeing Form
3.  Horseback Camp – Riding Waiver
